David Jubelirer M.D.

Board Certified Physician

Dr. Jubelirer is board certified physician specializing in the diagnosis and treatment of ADHD, Asperger’s Syndrome/Autism, and related psychosocial and academic problems for adults, adolescents, and children. He also performs and analyzes QEEG brain mapping , to better understand brain function and support an accurate diagnosis of ADHD or presence of traumatic brain injury.

In addition, Dr. Jubelirer also has expertise in evaluating problems with Executive Function skills, which primarily effect organization, time management, planning, self starting and prioritizing.

Rebecca Farnsworth, BSN, MSN, FNP-C

Family Nurse Practitioner

Rebecca is a certified Family Nurse Practitioner (FNP-C), earning a Bachelor of Science in Nursing from the University of Utah, a Master of Science in Nursing from Maryville University, and obtaining national certification through the American Association of Nurse Practitioners. She has over 23 years of experience in acute and primary care settings, including neuroscience, trauma, orthopedics, rehabilitation, pediatrics, and case management.

Rebecca has a passion for providing comprehensive and compassionate care, recognizing the importance of health promotion in fostering wellness across the lifespan. She actively engages in continuing education and professional development to stay abreast of evidence-based practices, developing individualized treatment plans. Rebecca has a holistic approach, encompassing physical health and psychosocial aspects such as emotional, social, individual beliefs, and cultural dimensions. She is committed to facilitating optimal outcomes for her patients, educating and engaging them in their health and well-being.

Annie Azuri, PMHNP

Psychiatric Mental Health Nurse Practitioner

Annie is a compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) specializing in the assessment and management of ADHD, as well as comprehensive mental healthcare across the lifespan. She earned her Doctor of Nursing Practice (DNP) degree in 2023, building upon years of experience as a Registered Nurse in pediatric and inpatient psychiatry.

Annie provides holistic, individualized care, excelling in thorough evaluations, tailored treatment plans, and medication management. By integrating evidence-based practices with a range of supportive interventions, she empowers her patients to foster lasting positive change in their lives, promoting greater focus and overall well-being.
