Effective June 1, we are no longer contracted with Cigna, United Healthcare, or UMR. Patients who would like to remain with our office may choose self pay at $135 per office visit.

If you plan to seek treatment elsewhere, contact our office and we can provide a medical records release to fax your records to your new provider.

Access is easy and there are plenty of parking spaces available.

1. Enter through the main lobby of the Regus Building and take the elevator to the 6th floor.
2. Turn left out of the elevator and check in at the reception desk. After checking in, you will be directed to our front desk staff, April and Alyssa. Bring your current insurance card and payment for copays, deductibles, or self-pay.
3. Once check-in is complete, you will be taken to a waiting room where your provider will meet you.
